ClosureRx collects information you provide directly, including your name, email address, phone number, practice details, state of licensure, and information shared during consultations with our team.

We also collect usage data including IP addresses, browser type, pages visited, and interaction patterns with our website and dashboard. This data helps us improve our services.

2. Protected Health Information (PHI)

During the practice closure process, we may handle Protected Health Information as defined by HIPAA. All PHI is encrypted at rest and in transit using AES-256-GCM encryption. We maintain strict access controls and audit logging for all PHI access.

We will execute a Business Associate Agreement (BAA) with all clients whose engagements involve handling PHI. Our data processing infrastructure is hosted on HIPAA-eligible services.

Information shared during consultations with our team is stored securely and used to provide personalized guidance. Our consultations do not constitute legal advice. Information shared during website intake or consultation forms may be used to create a lead record in our system. Ongoing engagement communications are treated as part of your client engagement.

5. Data Sharing

We do not sell your personal information. We may share data with service providers who assist in delivering our services (hosting, email, analytics), with your consent, or as required by law. All third-party service providers are bound by confidentiality obligations.

6. Data Retention

We retain client engagement data for the duration required by applicable regulations, typically 7 years for financial records. PHI is retained per the terms of your BAA and applicable state medical records retention requirements. You may request deletion of non-regulated data at any time.

7. Security

We implement administrative, technical, and physical safeguards to protect your information. This includes encryption, role-based access controls, audit logging, and regular security assessments. No system is 100% secure, and we cannot guarantee absolute security.
